They commented theyve been working hard to create a culture where people bring concerns forward and thats a national issue, thats not something thats limited to this particular hospital where this unfortunately happened, i think were all trying to work hard at this notion of seeing something and saying something and doing something.

The fact that there may be other patients, up to 26 other patients, really calls into question whether the culture of safety and reporting that theyre shooting for, whether theres more work that needs to be done.
